Upon discharge from the hospital or healthcare facility, the client or loved one can discuss the needed care from Stay Still Sitters as the client transitions back home. As the client transitions home, advanced planning will be completed to make sure Stay Still Sitters are prepared with such things as medications, doctor follow-up, nutrition, groceries purchased, and medical equipment or supplies needed provided by the client or loved ones. After the transition back home, the client or loved ones can meet with the owner of Stay Still Sitters to discuss or rectifiy any necessary changes or additions needed to be added to client's care. Also, make sure the house is prepared for a safe return.
