Rights and responsibilities:
- Designate a representative to be informed of/or participate in your care
- Be treated with dignity, courtesy, and respect
- Have client's property and belongings treated with respect
- Be free from verbal, mental, sexual, and physical abuse, including injuries of unknown source, neglect, and misappropriation of property
- Participate in and be informed in advance about any changes in care, including the completion of any necessary assessments to better determine care needs, decisions about care to be provided, revisions to the care plan, and the name of staff who will perform or supervise your care
- Expect to receive the services outlined in your plan of care in a reasonable and timely manner
- Be your voice (advocate)
- Expect confidentiality of all written and financial records and access to your records with a signed release from client
- Voice any grievances or concerns about your care provided by Stay Still Sitters without any fear of retaliation for voicing your grievances or concerns
- Receive competent care from Stay Still Sitters, regardless of race, color, national origin, religion, or sex
- Uphold proper ethics related to your care
As a client, I also understand I have the responsibility to:
- Maintain a safe and healthy environment for care to be given and protect my valuables by placing them in a safe place
- Engage in contributing to my care plan and making my request be made known if possible
- Provide supplies required for Stay Still Sitters to be equipped to carry out the services
- Notify Stay Still Sitters prior to the set schedule if client will not be available or does not need service at that scheduled time
- Treat the Stay Still Sitters with respect, courtesy, and dignity
- Pay for services agreed on in the service agreement or any additional services client has requested that are not included in the agreement
